Apply for plant design registration

An application for plant design registration is required before supplying the items listed in Part 1 of Schedule 5 of the Work Health and Safety Regulation 2011.

The designer or the person with management or control of an item of plant can apply to have a plant design registered. This could include the manufacturer, importer or supplier. To apply you will need to complete a Form 14 - Application for registration of plant design (PDF, 0.2 MB). The application form has seven sections and should be completed as follows:

  1. The applicant needs to complete sections 1, 2 and 3
  2. The designer complete sections 4 and 5
  3. The design verifier completes section 6 and 7

To ensure the application is complete and processed with minimum delay you should use the checklist listed below and on page 7 of the form.

To avoid delays in processing your application and confirm that your form is completely ready for submission, use this checklist.

Section 1. Details of applicant

  • Is this a modification to an existing Queensland design registration? If YES, have you stated the design registration number on the form?
  • Have you completed all the fields in the applicant details section of the form?

Section 2. Declaration of registration holder or applicant

  • Queensland is the only State in which you have lodged an application for registration of plant design. If not and the plant design is registered elsewhere, the application cannot be processed.
  • Has the applicant signed the declaration on page 2 of the form?

Section 3. Details of plant

  • Have you completed all the fields for details of plant including the subsections for the particular item of plant? For example, if the application is for a tower crane or mobile crane, complete the boxes in sections 3 and 3.3.
  • Have you attached a representational drawing? Note: the drawing should comply with AS 1100 – Technical drawing or an equivalent international standard for technical drawing. The drawing should be an arrangement drawing where the item depicted is the end product and includes dimensions.
  • Have you checked that the drawing number/s (including revision number/s) on the drawing/s provided are correct and match the drawing number/s stated by the designer and the design verifier?

Section 4. Designer statement

  • Has the designer completed all fields in section 4 of the form?

Section 5. Designer declaration

  • Has the designer signed and dated the declaration?

Section 6. Design verification statement

  • Has the design verifier completed all fields in section 6 of the form?
  • Has the design verifier documented the process used to verify the design? Note: this document is not required to be submitted with the application but must be available if requested.
  • Did more than one person participate in the design verification? If so, ensure a design verification statement has been provided by each person.
  • Is the design verifier a Registered Professional Engineer of Queensland (RPEQ) and has the registration number been provided?

Section 7. Declaration of design verifier

  • Has the design verifier signed and dated the declaration?

The same process applies to a new plant design registration or for an altered plant design.

*To alleviate cost-of-living pressures the government has set the Government Indexation Rate for 2024–25 to 0.00% for many government fees and charges. This means that all Office of Industrial Relations fees and charges will be unchanged in 2024-25 from 01 July 2024 to 30 June 2025.

Submit your application

To submit your application, email the completed form to designregistrationapplications@oir.qld.gov.au.

You will need to include any referenced drawings with the application and the fee is required with the application.

Payment options

You must pay the application fee when lodging the application otherwise applications will not be processed. GST does not apply to this fee.

  • Credit card - A payment link will be sent from: no-reply@bpoint.com.au once your application has been submitted. The subject of the email will include “OIR – High Risk Work Lic – Design – Payment Request”. What happens after I submit an application?

After receiving an application and payment has been receipted:

  1. We will check each section of application form. It is important that all applicants provide the correct information to ensure there are no delays.
  2. If any information is missing, we will contact the applicant by email or phone.
  3. Once details are verified the application will be registered.
  4. A registration document will be returned to the applicant and a copy kept by WHSQ. The plant design registration number will be provided on the document, which includes a confirmation section completed by WHSQ. On receipt of the confirmed design registration, the applicant must provide the registration number to anyone involved with the manufacture, importation or supply of the plant. Manufacturers, suppliers and importers are required to provide the registration number to anyone who purchases the plant.
  5. Following the registration of plant design, an owner is required to register specified items of plant before it is permitted to be used at the workplace. Find out more about plant registration.
