    Fatal incident involving farm machinery

    Earlier this year, a worker sustained fatal crush injuries undertaking fencing activities on a rural property. Early investigations found that the tractor equipped with a ram and auger setup at the rear was being used for the digging of holes and post installation. It seems following the completion of several holes the ram and auger configuration disengaged unexpectedly and fell on the worker.

    In a recent incident, a worker suffered serious injuries after being struck by a forklift carrying a pallet of stock inside a warehouse. Initial enquiries indicate the worker was walking through a section of the warehouse when, for reasons yet to be established, the person was struck by the moving forklift. Investigations are continuing.

    A worker suffered serious injuries after falling from a second floor balcony to the ground at a construction site. An edge protection system was installed. Early investigations indicate there may have been an issue with the system when the worker was disposing of building waste from the balcony area.

    Workers sustain serious injuries from two separate conveyor incidents

    In 2024 an incident occurred where a worker was using a shovel to clear out a build-up of dirt from around the bottom idler roller of the main conveyor. The worker placed the shovel inside the conveyor where it has grabbed and drawn his arm into the opening of the unguarded conveyor causing his arm to be amputated. In a separate incident, a worker assisting to recommission a plastics extruder machine had his right arm caught in a system of rollers. The worker sustained serious fracture injuries to his arm.

    A tractor has rolled causing fatal crush injuries to operator

    In a recent incident, the operator of a tractor has sustained fatal crush injuries when the tractor rolled. Initial enquiries have revealed that the tractor being operated had a foldaway rollover protective structure that was folded in half at the time the tractor rolled. It seems that the operator was attempting to execute a right hand turn on an apparent 20 degree slope causing the tractor to roll.

    Working remotely and isolated

    In a recent incident, a worker was found deceased after attempting to seek assistance when the vehicle he was travelling in became bogged. Early investigations have found that two workers were travelling along outback roads in two vehicles relocating equipment. It appears that at some point they have turned off the bitumen road onto a dirt road where they encountered water across the road. It seems the vehicles became bogged and after failed attempts to get the vehicles out, the workers decided to travel on foot to seek help. It is believed that the temperature was approximately 38-40 degrees. After some time, one of the workers decided to turn back to the vehicles and was subsequently found and taken to the nearest homestead. The other worker was found deceased two days later.

    Two recent incidents involving the use of high-pressure water spray equipment on asbestos roofs have resulted in asbestos contaminated dust or debris (ACD) being spread across each of the sites in addition to neighbouring properties. Investigations into both incidents are continuing.

    Two workers injured in fire and explosion at boatyard

    In a recent incident, two workers suffered serious burns as a result of an explosion and fire at a marine facility. Initial enquiries indicate both workers were installing inboard marine engines into the floor of a boat located on a hardstand at the time. At some point during the installation process, the workers undertook testing of the engines using an auxiliary fuel tank as it appears the main petrol tanks were disconnected. At some point during the testing process, for reasons yet to be established, an explosion and fire occurred resulting in serious burns to both workers. Investigations are continuing.

    Bridge and gantry crane serious incidents

    In September 2023, two young workers were injured when a bundle of steel lengths weighing over one tonne fell from a bridge crane (overhead travelling crane). Initial investigations indicate a lifting chain slipped off one end of the bundle of steel while it was being moved using a dual hoist bridge crane inside a warehouse. The bundle fell, striking a worker and a second worker was injured while assisting the first. In October 2023, a worker suffered serious leg and foot injuries after being struck by a gantry crane wheel. Initial enquiries indicate he was using a jackhammer close to the crane’s rail system when the crane began operating and one of its wheels struck him.
