    Fixed machinery incidents, including two fatalities

    In May 2022, a worker suffered serious de-gloving injuries when his hand was trapped in a ‘curtain coating roller’ machine. At this stage of the investigation, it’s unclear exactly what the worker was doing, but it appears his hand was drawn in between the machine’s two rollers.

    Falls into ground level holes

    In March 2022, a landscaping trainee suffered serious foot and spinal injuries when he fell approximately 7m into the opening of a sewer access pipe. Initial enquiries indicate several trainees were doing a landscaping course that involved using hedgers, whipper snippers and mowers. The work area the trainees were preparing to clear was overgrown with grass. After a site inspection, the trainees commenced clearing the area when for reasons yet to be established one of them fell into the pipe. In April 2022, a worker was left with bruises and abrasions after falling into a pile hole around 4m deep. Early enquires suggest several workers were inspecting the holes dug as part of the construction of a sound barrier when one of them removed a plywood cover to take a closer look. The worker then fell into the open hole. Investigations are continuing.

    Worker’s hand partially amputated by a saw

    In April 2022, a worker suffered a partial amputation of his hand after it contacted a moving saw blade approximately 1m in diameter. Early investigations revealed it was a breakdown saw used to mill trees. It appears the worker was cutting a large timber log on the saw when he has accessed another blade to clear or dislodge an obstruction. Unfortunately, his hand has come into contact with the moving operational blade, causing significant amputation injuries. Investigations into the incident are continuing.

    Worker struck in abdomen with stun gun

    In February 2022, a worker suffered serious injuries after being struck in the abdomen by a loaded captive bolt stun gun (also known as cattle gun, bolt gun, stun gun or stunner). Early investigations revealed the worker was part of a maintenance team and was cleaning the loaded captive bolt gun when it fired.

    Four injured in workplace explosion

    In November 2021, one worker was seriously injured while three other workers received minor injuries following a workplace explosion and fire. Early enquiries indicate an “extractor” has exploded. This machine is used to extract rosin and turpentine from pine wood chips by spraying hot turpentine over the wood chips in a process known as solvent extraction. For reasons yet to be established, an explosion has occurred. Investigations are continuing.

    Quad bike fatality on a rural property

    In November 2021, a rural property owner died after appearing to roll his quad bike. Initial enquiries suggest the man was in all likelihood mustering cattle on his own in a slopey area that also appeared to be sodden and boggy at the time. Investigations indicate the quad bike had been fitted with an Operator Protection Device (OPD) by the manufacturer. For reasons unknown, it appears the OPD had been removed with the upper mounting bracket remaining.

    One worker killed and six injured when powerline arced to harvester

    In July 2021, one worker died and six others received electric shocks when a harvester came close enough to an overhead powerline to cause an electric arc. Initial investigations found the powerline had dropped approximately 3.6m from its original height because the stay wire on one of the poles had broken, potentially due to corrosion.

    In July 2021 a worker sustained multiple fractures and internal injuries after he fell approximately six metres from the platform of a truck mounted elevating work platform while trimming tree branches.

    Worker killed by falling branch while felling trees

    In July 2021, a worker was felling trees in a forest plantation when he was struck and fatally injured by a tree limb. It appears the tree the worker was felling struck a separate dead tree and a branch from that tree fell and struck him.

    Truck driver exposed to phosphine gas

    In July 2021, a truck driver began feeling very unwell after transporting grain to a feedlot with his semi-trailer. It appears the worker was exposed to a fumigant that produced phosphine gas which is commonly used to control insect infestations in grain.

    In April 2021, a contractor suffered multiple fractures, degloving and severe lacerations to his arms when he was trapped in a wetting auger at a feedlot.

    Workers struck by mobile plant

    In January 2021, a worker on a private property was fatally injured when a telehandler ran over him. Early investigations show the telehandler was loading a crop-dusting plane with fertiliser at a private airstrip on the property when the operator reversed it over a worker.
