    Worker fatally injured by chain

    In February 2023, a forklift operator died after he was struck by a chain at a building material supply workplace. Early investigations indicate a forklift was unloading steel weld mesh from a shipping container using a connection point located in the forklift’s counterbalance to drag the material out of the container. It appears part of the towing equipment failed and the chain recoiled towards the forklift, striking the operator.

    In November 2022 a worker was seriously injured when he was struck by an excavator bucket at a construction site. Initial enquiries indicate the excavator was operating near the worker when, for reasons yet to be established, its bucket unexpectedly detached from a quick-hitch on the dipper arm, striking him.

    Workers injured by front-end loader bucket

    In July 2022, two workers were injured, one seriously, after being struck by a front-end loader bucket while working as part of a road maintenance team. Early enquiries indicate the two workers were attaching an aggregate spreader or ‘cockerel box’ to the rear of a tipper truck. The aggregate spreader was being manipulated into place using chains attached to the bucket of the front-end loader. After the workers attached the aggregate spreader to the truck, the front-end loader’s bucket unexpectedly detached striking both workers.

    In October 2022, a worker suffered serious burns to his upper body after being engulfed by a fire at an engineering workplace. Early investigations revealed he was welding a metal plate to repair a leak to a tank, when an explosion occurred which resulted in a fire.

    In June 2022, several concreters were injured after falling between three and four metres when a formwork deck collapsed at a construction site. Initial investigations indicate they were pouring a concrete slab for a mezzanine floor when for reasons yet to be established, the formwork system failed.

    Worker’s leg crushed by steel sheet dropped from crane

    In August 2022, a worker’s leg was crushed by a steel sheet weighing approximately four tonnes. Investigations found he was attempting to move the sheet of steel from the back of a flatbed truck with an overhead travelling crane and lifting magnet attachment. For reasons not yet established, the sheet of steel became dislodged from the crane and dropped.

    Young worker struck by heavy timber sheets

    In September 2022, a 17-year-old work experience student’s pelvis was fractured when a pack of heavy timber sheets fell onto him. The sheets were stacked on their edge and leaning against a wall. Early investigations found the boards weighed approximately 30kg each and the student was retrieving a cover board from the rear of the pack.

    In July 2022, a worker was seriously injured when he fell over 3m from a roof. Early investigations indicate he was removing asbestos roofing sheets and was lying on the roof facing downward when his body slipped and he fell head first off the roof.
