Fatal forklift incidents
In July 2019, a person was fatally crushed by a portable generator being unloaded from a truck by a forklift. In a separate incident in May 2019, a worker was fatally crushed between a reversing forklift and a truck at a transport yard. Investigations into both are continuing.

Worker seriously burned in vehicle gas tank fire
In April 2019, a worker was seriously burned while attempting to decommission a liquefied petroleum gas (LPG) tank from a motor vehicle at a workplace. It appears he was removing screws on the LPG tank using a battery-operated screwdriver which created an ignition source and started a fire.

Young worker injured by exploding tyre rim
In May 2019, a young worker suffered serious facial injuries when the split rim that he and another worker were performing maintenance on, exploded. For reasons yet to be established, it appears the tyre exploded while the workers were removing a tyre from a split rim on an earthmover. Investigations are continuing.

Automotive mechanic struck by vehicle parts
In March 2019, a worker suffered serious head injuries when a suspension air bag assembly ejected under pressure from a heavy vehicle in a mechanical workshop. Early investigations indicate the vehicle was raised on a hoist and the worker was underneath along with other workers disconnecting and removing suspension parts to allow access to a hard to reach spot in preparation for other maintenance tasks.

Worker fatally crushed by steel pipe
In December 2018, a worker died when he was crushed by a steel pipe which fell off the back of a truck. The worker and two others were unloading two 12m steel pipes, each weighing 1.8 tonnes, one at a time with a forklift. After the ratchet straps were released, it appears one of the pipes rolled, striking the stanchions and causing them to fail. The pipe continued rolling and fell off the truck, landing on and crushing the worker’s chest and head. In January 2019, workers were loading large pipe tees (a T-shaped pipe junction) weighing 500kg onto a trailer. The workers identified that the strapping of the tees onto timber pallets was not adequate for the size and weight of the load. One worker climbed onto the trailer to place additional strapping on one tee. While he was still on the trailer, a high reach forklift operator loaded another pipe tee onto the trailer. The tee slid forward off the forklift tines and struck him, knocking him off the trailer and continuing to fall off

Worker’s lower leg amputated as forklift strikes bollard
In September 2018, a forklift operator’s lower leg was amputated when the forklift he was driving struck a bollard. Initial investigations suggest he was driving the forklift around a corner of a packing hall onto an exit ramp and his foot may have been outside of the manufacturer’s designated operating position. Investigations are continuing.
