Three serious forklift incidents – one fatal
In February 2022, a worker suffered serious leg injuries after being struck by a forklift. Early investigations suggest the man was walking next to the forklift to prevent a load of glass panels from swinging. In March 2022, a worker suffered fatal injuries when he was struck by a forklift, which early investigations indicate was moving and carrying a load. In May 2022, a worker suffered serious injuries to his lower legs when he was struck by a counter-balanced forklift. Early investigations show the worker was walking behind the loading dock area of the warehouse when the forklift reversed out of a truck into the warehouse and struck him. Investigations are continuing.

Skid steer loader incidents
In April 2021, a worker was killed when he was struck by the skid steer loader he was operating. Early investigations indicate the skid steer loader was being used to repair a residential driveway and it struck the worker after he fell out of the machine.

Workers struck by mobile plant
In January 2021, a worker on a private property was fatally injured when a telehandler ran over him. Early investigations show the telehandler was loading a crop-dusting plane with fertiliser at a private airstrip on the property when the operator reversed it over a worker.

Worker falls from forklift
In December 2020, a worker suffered serious injuries when he fell from a forklift. Early investigations indicate he was attempting to repair a jammed roller door. The man was working from a stillage container being used as a makeshift work box and had been lifted on the tines of a forklift. It appears that as the worker moved to one side of the crate it overbalanced causing him (and the crate) to fall almost 5 metres.

Worker fatally injured attempting to jump start a tractor
In December 2020, a worker suffered serious injuries when he fell from a forklift. Early investigations indicate he was attempting to repair a jammed roller door. In December 2019, a worker suffered serious injury after falling approximately 5m through a roof whilst doing maintenance on a shed. In a similar incident in February 2020, a man died after falling through the roof of a building. Early investigations indicate he was repairing a skylight.

Crush incidents when loading/unloading
In January, an interstate truck driver suffered multiple leg fractures when a stone slab fell off the back of a truck. It appears the stone was secured to an ‘A’ frame with straps. Early investigations indicate that when the driver removed the straps, the stone slabs toppled and struck him. In February, a truck driver suffered severe crush injuries when a pack of timber boards being delivered to a timber yard fell onto him. Early investigations indicate the pack of timber caught onto an adjacent one when being unloaded. It caused the strapping to snap and the timber fell onto the driver.
