    Working near, or on, tip trucks and tipper trailers

    The purpose of this safety alert is to highlight the risk of crushing or fatal injuries due to overturning tip trucks, tipper trailers or uncontrolled movement of tipper trays. The term “tipper” will be used to describe tip trucks and dump trucks that operate on and off road and includes trailer tippers. All tippers, including those that tip in line with the truck body and side tippers, are covered by this alert.

    Two incidents involving young workers and trailers

    Recently, a young worker was fatality injured when a trailer separated from a towing vehicle pinning them between the trailer and a structure. Early investigations indicate the worker was assisting with spotting the trailer while it was being reversed down an incline. For reasons yet to be established, the trailer became separated from the towing vehicle. In a separate incident, a young worker was seriously injured after they were struck by a wood chipper trailer. Early investigations indicate the worker had been assisting with the retrieval of the wood chipper after truck it was attached to became bogged.

    Working safely on guyed towers

    The purpose of this safety alert is to highlight the risk of fatal injury when guyed lattice towers fail due to inadequate guy wire support. These towers are commonly used as radio towers or for the support of metrological equipment. The safety alert will detail the importance of risk assessment of the construction process and the need to design suitable risk control measures based on the hierarchy of control. For the purposes of this alert the term “guy” will be used to describe a guy wire, also known as a stay or stay wire.

    This year has seen a number of people die as a result of snorkelling activities. To date, there have been five fatal incidents involving people over 50 participating in recreational snorkelling activities offered by tour companies.

    Fatal incident involving farm machinery

    Earlier this year, a worker sustained fatal crush injuries undertaking fencing activities on a rural property. Early investigations found that the tractor equipped with a ram and auger setup at the rear was being used for the digging of holes and post installation. It seems following the completion of several holes the ram and auger configuration disengaged unexpectedly and fell on the worker.

    Worker killed in elevating work platform incident

    In a recent incident, a worker suffered fatal crush injuries while operating an elevating work platform (EWP). Initial enquiries indicated the worker became trapped between the top rail of the EWP basket and a panelled bulkhead above.

    The purpose of this safety alert is to remind duty holders of the need to prepare and follow detailed lifting procedures; especially when using synthetic lifting slings, slings in basket hitch configuration or when carrying out a single lift with more than one crane.

    In a recent incident, a worker suffered serious injuries after being struck by a forklift carrying a pallet of stock inside a warehouse. Initial enquiries indicate the worker was walking through a section of the warehouse when, for reasons yet to be established, the person was struck by the moving forklift. Investigations are continuing.

    Worker seriously injured in fall from balcony

    A worker suffered serious injuries after falling from a second floor balcony to the ground at a construction site. An edge protection system was installed. Early investigations indicate there may have been an issue with the system when the worker was disposing of building waste from the balcony area.

    Workers sustain serious injuries from two separate conveyor incidents

    In 2024 an incident occurred where a worker was using a shovel to clear out a build-up of dirt from around the bottom idler roller of the main conveyor. The worker placed the shovel inside the conveyor where it has grabbed and drawn his arm into the opening of the unguarded conveyor causing his arm to be amputated. In a separate incident, a worker assisting to recommission a plastics extruder machine had his right arm caught in a system of rollers. The worker sustained serious fracture injuries to his arm.
