    Excavation face and tilt-up panel collapse

    This safety alert highlights the risk of collapse of excavation faces and the associated risk of collapse of surrounding structures. The principles in this alert can also be applied to any natural earth or rock face where there is a risk of collapse and workers may be at risk (i.e. a structure being constructed next to a natural face that may be unstable).

    Excavator bucket strikes construction worker

    In November 2022 a worker was seriously injured when he was struck by an excavator bucket at a construction site. Initial enquiries indicate the excavator was operating near the worker when, for reasons yet to be established, its bucket unexpectedly detached from a quick-hitch on the dipper arm, striking him.

    Electricity meter contractor fatally attacked by dogs

    In December 2022, a contractor was fatally injured when he was attacked by a number of dogs. Preliminary investigations found he was attending a residential address to check an electricity meter. It appears that the dogs attacked after he opened the first gate and entered the property.

    Worker killed in farm ute rollover

    In December 2022, a farm worker was fatally injured while travelling as a passenger in a farm ute. Early investigations indicate the worker was sitting in the passenger seat when for reasons yet to be established, the driver lost control and the vehicle rolled over.

    Worker sustained fatal injuries from tractor rollover

    In November 2022, a worker sustained fatal crush injuries when he was ejected from a tractor and trapped underneath as it rolled over. Early investigations indicate that the worker was not wearing a seat belt when driving the tractor along a road while hauling an empty sugar cane bin. For reasons yet to be established, the tractor has overturned and rolled onto its roof. Investigations are continuing.

    In October 2022, a young worker was killed after being struck by the road roller he had been operating. For reasons yet to be established, it appears the young worker has hopped off the road roller and was then struck by it.

    Workers injured by front-end loader bucket

    In July 2022, two workers were injured, one seriously, after being struck by a front-end loader bucket while working as part of a road maintenance team. Early enquiries indicate the two workers were attaching an aggregate spreader or ‘cockerel box’ to the rear of a tipper truck. The aggregate spreader was being manipulated into place using chains attached to the bucket of the front-end loader. After the workers attached the aggregate spreader to the truck, the front-end loader’s bucket unexpectedly detached striking both workers.

    Serious fire and explosion injuries in three hot work incidents

    In October 2022, a worker suffered serious burns to his upper body after being engulfed by a fire at an engineering workplace. Early investigations revealed he was welding a metal plate to repair a leak to a tank, when an explosion occurred which resulted in a fire.

    Workers injured in formwork collapse

    In June 2022, several concreters were injured after falling between three and four metres when a formwork deck collapsed at a construction site. Initial investigations indicate they were pouring a concrete slab for a mezzanine floor when for reasons yet to be established, the formwork system failed.

    Worker’s leg crushed by steel sheet dropped from crane

    In August 2022, a worker’s leg was crushed by a steel sheet weighing approximately four tonnes. Investigations found he was attempting to move the sheet of steel from the back of a flatbed truck with an overhead travelling crane and lifting magnet attachment. For reasons not yet established, the sheet of steel became dislodged from the crane and dropped.
