Workers killed by mobile plant
In July 2022, a worker died after being hit by a moving water truck. The man was part of a crew working on a development road. Early investigations indicate he was struck when the water truck was reversing. In June 2022, a skid steer loader was spreading gravel onto a residential driveway when for reasons yet to be established, it struck and killed a person who was close by. Investigations are ongoing.

Worker fatally struck by vehicle
In May 2022, a station hand was trapped between the vehicle he’d been driving and an access gate. Early investigations found the worker was checking water bores when he arrived at an access gate. It appears he put the work supplied four-wheel drive into low range before exiting the ute to open the gate. Tragically, the four-wheel drive vehicle has crept forward, fatally pinning him against the access gate.

Worker crushed by tractor
In May 2022, a worker was crushed to death by a tractor. Early investigations indicate the man was operating the tractor (with slasher attachment) by himself. It appears he either got off the tractor while it was moving or fell from the tractor and it struck him.

In May 2022, a worker suffered serious de-gloving injuries when his hand was trapped in a ‘curtain coating roller’ machine. At this stage of the investigation, it’s unclear exactly what the worker was doing, but it appears his hand was drawn in between the machine’s two rollers.

Fatal roadside incident
In April 2022, two motor mechanics were repairing a broken-down bus on the roadside. Tragically, the bus was struck by a passing vehicle and both men were killed. Investigations are continuing.

Mobile crane boom failure and major inspections
The purpose of this safety alert is to highlight the risk of a potential catastrophic failure of corroded bolts on amusement rides. During inspection of teacup (cup and saucer) rides the bolts attaching the patron carriage (teacup) were found with severely corroded bolts. The severely corroded mounting bolts cannot be identified without being removed for inspection. The information in this alert applies to bolted joints on any amusement devices that have limited access for inspection.

Young worker suffers cryogenic burns
A young worker at a heavy engineering maintenance facility suffered serious cryogenic burns after dunking his hands in liquid nitrogen whilst attempting to shrink a brass bush to put into an excavator boom arm. Investigations are continuing.

Mobile crane wire hoist rope failure
In May 2022, a worker suffered facial injuries when a steel beam fell from a mini-crawler crane at a construction site. Early investigations indicate the mini-crawler crane was lifting the beams while the worker was using a tag line when the crane’s wire rope failed. As a result, the steel beam fell approximately 5m hitting several precast props before striking the man’s face. Investigations are continuing.

Worker fatally crushed unpacking shipping container
In April 2022, a worker was crushed to death after being trapped between a triangular stacked (set of three) semi-trailer axels (weighing approximately 1 tonne) and the side of a shipping container. Early investigations show the man was unpacking product from the shipping container when for reasons not yet established the axels moved and trapped him.

Forklift gas bottle ignites injuring two workers
In April 2022, two workers suffered burns when gas ignited while they were refilling a forklift gas bottle. Early investigations indicate a worker was refilling a forklift gas bottle from a larger tank when it appears they had difficulty with a nozzle. Another forklift operator came to assist. For reasons yet to be fully established, the gas ignited causing minor burns to one worker and 2nd and 3rd degree burns to the other. Investigations are continuing.

Falls into ground level holes
In March 2022, a landscaping trainee suffered serious foot and spinal injuries when he fell approximately 7m into the opening of a sewer access pipe. Initial enquiries indicate several trainees were doing a landscaping course that involved using hedgers, whipper snippers and mowers. The work area the trainees were preparing to clear was overgrown with grass. After a site inspection, the trainees commenced clearing the area when for reasons yet to be established one of them fell into the pipe. In April 2022, a worker was left with bruises and abrasions after falling into a pile hole around 4m deep. Early enquires suggest several workers were inspecting the holes dug as part of the construction of a sound barrier when one of them removed a plywood cover to take a closer look. The worker then fell into the open hole. Investigations are continuing.
